Key Responsibilities: Client Assessments: Conduct thorough assessments to understand clients’ needs, goals, and strengths, using a person-centered approach. Care Planning: Develop individualized care plans with clients, families, and other professionals to ensure comprehensive support. Service Coordination: Help clients access healthcare, housing, education, employment, and other crucial resources. Advocacy: Stand up for clients’ rights and ensure they have access to the services they deserve. Monitor Progress: Track client progress, adjusting care plans as needed to ensure the best possible outcomes. Crisis Support: Provide intervention and support during times of crisis, working alongside emergency services when necessary. Documentation: Maintain accurate, confidential records of client interactions, services provided, and progress. Collaboration: Work with community organizations, government agencies, and others to address challenges and improve service delivery. Stay Informed: Keep up to date with the latest laws, regulations, and best practices in disability services and homelessness prevention. Qualifications: Education or Experience: A Bachelor’s degree in social work, psychology, counseling, or a related field, or at least 5 years of relevant work experience (or a combination of both). Interpersonal Skills: Ability to connect with and build trust among diverse populations. Organizational Skills: Strong ability to manage time, prioritize tasks, and pay attention to detail. Tech Proficiency: Experience with Microsoft Office and electronic health record systems. Reliable Transportation: A valid driver’s license and dependable transportation are required for client visits. What’s in It for You?
