The Community Engagement Nurse takes a pivotal role within our team, dedicated to providing exceptional care, closely monitoring health conditions, administering medications, and empowering patients and families through education on disease processes and self-care. You will forge meaningful relationships with patients, guiding them through their unique medical and social challenges, collaborating with physicians and various team members to enhance patient outcomes.

Key Responsibilities:

  • Deliver outstanding customer service to patients and team members.
  • Develop, implement, evaluate, and document comprehensive nursing plans of care.
  • Perform clinical tests and procedures, administering medications within your nursing scope.
  • Monitor patients for any adverse reactions and take appropriate actions per established protocols.
  • Analyze patient data to inform critical decision-making for care.
  • Educate patients and their families on treatment plans and health conditions.
  • Support patient independence by helping set care goals and teaching essential self-care skills.
  • Ensure accurate documentation of nursing services in patient records.
  • Monitor specialized equipment, ensuring compliance with protocols and reporting findings.
  • Provide compassionate emotional support to patients and their families.
  • Uphold safety and infection-control standards, along with medication administration protocols.
  • Maintain and manage inventory of nursing supplies, placing orders as necessary.
  • Ensure equipment functionality through routine checks and maintenance.
  • Perform any other responsibilities as assigned by management.

  • Fluency in English is required.
  • Some travel between centers may be necessary to support patient care requirements.

Education and Experience:

  • High school diploma or equivalent is mandatory.
  • Graduation from an accredited nursing program is required.
  • A valid and active RN license in the state of employment is essential.
  • Compact License is preferred in eligible states.
  • A minimum of one (1) year of nursing experience is required.
  • Three (3) years of overall clinical experience preferred.
  • Basic Life Support (BLS) certification is required within 90 days of hire.
  • A valid driver’s license is necessary for this role.
  • Experience with geriatric patients is desirable.

Compensation:

$35.8 - $51.17 Hourly
